This article details how to complete the Insurance / Injury / Illness tab on the Safety Profile.
To successfully complete your profile and generate a safety score, you will need to update the information beneath the Insurance/Injury/Illness tab. This article is relevant to all US contracting partners. If you are International, please visit this article here.
Global, National or Regional Data
Please identify if the Occupation Safety Data entered in your profile is global or if it is regional. If it is regional data, enter the associated region.
Global data is a representation of your data on a global scale.
National data is a representation of your company data for all of your sites within one country.
Regional data is representing the data of the local branch of your company in that specific region.
The following information must be provided for each of the past 3 years shown below. As a new year begins, the oldest year on your profile, if longer than 3 years ago, will not be required or reflected in your score.
Did Your Company Perform Work This Year: Check yes or no for each of the past 3 years. When you click ‘yes’ the fields will open up to allow for data entry and uploads.
OSHA Recordable Cases: Enter the TOTAL number of OSHA recordable cases that you had during the course of each of the past 3 years.
DART Cases: Enter the number of cases that resulted in Days Away from Work, Restricted Duty, or Job Transfer. (Do not enter the number of days away from work or the number of days of restricted duty or job transfer)
If you have questions about how to classify an injury visit the record keeping page on osha.gov. Not all injuries are recordable.
Total Days Away From Work: Enter the total number of days employees were away from work because of a DART case.
Total Days of Job Transfer or Restricted Duty: Enter the total number of days employees were on a job transfer or restricted duty because of a DART case.
- Fatalities – If you experience any incidents that resulted in a fatality enter them here.
- Employee Hours Worked – Enter the total number of employee hours worked during each of the past 3 years (do not include subcontractor hours). The number of hours entered in each field should match the number of hours on the OSHA 300A summary form for the same year.

EMR: You can get your Experience Modification Rating or EMR from your Worker’s Compensation Insurer. EMR is a rating based on your worker’s compensation claims history and involves a review of several claims-related factors.
Upload EMR Documentation for each of the past 3 years.
For EMR documents, contact your worker’s compensation insurance agent and request a letter or document on the insurance agent’s letterhead that identifies your EMR for the given year. Three (3) separate documents must be uploaded here. If you have one document that contains multiple years, upload the same document to each year.

EMR - Experience Modification Rate
EMR is a rating calculated by the insurance industry that involves a detailed review of the cost associated with past injuries and analysis of future risk.
To obtain your EMR contact your workers compensation insurance broker/provider. For EMR documentation request a letter or document from your workers compensation insurance broker/provider that displays your EMR for each of the last three years/coverage periods. You may choose to provide one document that displays all three years/periods and upload it three times or provide individual documents for each year.
If your company does not have an EMR either because it is a new company or because the company does not qualify for an EMR enter 1 for each year. Then upload a letter for each year explaining the reason that the company does not have an EMR.
